Where to mail your U.S. passport application (DS-82, DS-5504, DS-64)

The official mailing addresses for mail renewals, name changes and loss reports, why you never mail Form DS-11 yourself, and how to send your envelope safely.

Form DS-82: renewal by mail

First check that you can renew by mail.

You can renew by mail with Form DS-82 only if every one of these conditions is true.1

  • You can send your most recent passport with the application.
  • It was never reported lost or stolen.
  • It was issued within the last 15 years.
  • It was issued when you were 16 or older and was valid for 10 years.
  • It is in your current name, or you can show a marriage certificate, divorce decree or court order for the change.
  • It is not damaged beyond normal wear and tear.

Form DS-5504: name change or error correction

If less than 1 year has passed since both your passport was issued and your name legally changed, mail Form DS-5504 with your most recent passport, the original or certified name change document and one photo.6 To fix an error in your name, sex or place of birth, or a printing defect, mail Form DS-5504 with the passport, one photo and evidence of the error. Reported within 1 year, the new passport gets full validity; after that, it keeps the original expiration date.7

Form DS-64: reporting a lost or stolen passport only

Report a valid lost or stolen passport right away: online (cancelled within 1 business day), by mail with Form DS-64, or together with a new Form DS-11. Reporting does not replace it, and a reported passport cannot be used even if found. Do not report an expired passport.10 ATTN: CLASPU.S. Department of State44132 Mercure CirP.O. This address is only for the report. It does not get you a new passport. To replace it, you apply in person with Form DS-11: see Lost or stolen passport.

Form DS-11: you do not mail it

You apply in person with Form DS-11 in any of these situations:

You must apply in person at an acceptance facility with Form DS-11 in any of these situations.12

  • It is your first U.S. passport (age 18 or older).
  • Your last passport was lost, stolen or damaged.
  • Your last passport was issued more than 15 years ago.
  • Your last passport was issued when you were under 16.

You take Form DS-11 to an acceptance facility, often a post office, library or local government office, and hand it in there. You do not put it in the mail yourself. Do not sign Form DS-11 until the acceptance agent asks you to; the agent gives you an oath and watches you sign.13 Find a facility at iafdb.travel.state.gov14.

Living outside the United States

From outside the United States and Canada, the domestic mailing addresses do not apply; contact the nearest U.S. embassy or consulate.15

Step by step

  1. Confirm which form you are sending: DS-82, DS-5504 or DS-64. If it is DS-11, do not mail it.
  2. Choose routine or expedited service. For expedited service, write "EXPEDITE" in large letters on the outside of the envelope.
  3. Find the address for your form, your state and your service speed above, and copy it exactly, including the full ZIP code.
  4. Put everything in one envelope: the signed form, the photo, your passport and documents, and the check or money order.
  5. Mail it with the U.S. Postal Service, using a service with tracking.
  6. Keep a copy of everything you sent and your mailing receipt.
  7. Wait for the first status update, then follow your application (see Track your passport application).

  • "EXPEDITE" written on the outside, if I pay for expedited service
  • Form signed and dated
  • Photo, passport, documents and payment inside, as the form asks
  • A copy of everything kept at home
  • Sent with the U.S. Postal Service, with tracking

Do and don't

Do

  • Copy the address exactly as it appears here, line by line.
  • Use an envelope big enough that you do not have to fold the photo.
  • Keep the tracking number until your passport and documents are back.

Don't

  • Don't mail Form DS-11 yourself; it is handed in at an acceptance facility.
  • Don't send a renewal to the Form DS-64 address, or a loss report to a renewal address.
  • Don't use an expedited address unless you are paying for expedited service.
  • Don't use these addresses if you live outside the United States and Canada.

By mail or at an acceptance facility, pay the application fee with a check or money order payable to "U.S. Department of State", with the applicant's name and date of birth in the memo. The acceptance fee is paid separately to the facility. Online renewals are paid by credit or debit card.20

How long mail takes

Frequently asked questions

Can I send my renewal with FedEx or UPS?

The official instructions say to mail Form DS-82 with the U.S. Postal Service. Print Form DS-82 single-sided, sign and date it, staple one photo with four staples in the corners, include your most recent passport and the fee, and mail it with the U.S. Postal Service. No $35 acceptance fee applies to DS-82.5

Where do I mail Form DS-11?

Nowhere. You hand it in at an acceptance facility. Do not sign Form DS-11 until the acceptance agent asks you to; the agent gives you an oath and watches you sign.13

What do I write on the envelope?

The address for your form and service speed. If you pay for expedited service, also write "EXPEDITE" on the outside of the envelope.

How do I know my application arrived?

Use the tracking number from your mailing receipt. It can take 2 weeks22 from the day you mail it until the first status appears on passportstatus.state.gov23.
