Recent passport and recent name change

If less than 1 year has passed since both your passport was issued and your name legally changed, mail Form DS-5504 with your most recent passport, the original or certified name change document and one photo.3

The government charges no fee1 for this with routine service. Expedited service costs $604 extra.

More time has passed

After more than 1 year, renew by mail with a certified copy of the name change document if you qualify; otherwise apply in person with Form DS-11.7

You can renew by mail with Form DS-82 only if every one of these conditions is true.8

  • You can send your most recent passport with the application.
  • It was never reported lost or stolen.
  • It was issued within the last 15 years.
  • It was issued when you were 16 or older and was valid for 10 years.
  • It is in your current name, or you can show a marriage certificate, divorce decree or court order for the change.
  • It is not damaged beyond normal wear and tear.

Without a court order, divorce decree or marriage certificate, apply in person with Form DS-11 and Form DS-60, completed by 2 people who have known you by both names, plus 3 public records showing you used the new name for 5 years or more.9

This route takes more preparation than the others. If your situation is unusual, check the official source before you start.

Fixing a data or printing error

To fix an error in your name, sex or place of birth, or a printing defect, mail Form DS-5504 with the passport, one photo and evidence of the error. Reported within 1 year, the new passport gets full validity; after that, it keeps the original expiration date.10

The government charges no fee2 to correct it while your passport is still valid. Form DS-5504 goes to the same addresses shown above.

Step by step

  1. Find the issue date on your passport and the date on your marriage certificate, divorce decree or court order.
  2. Compare them with the rules above to choose your route: Form DS-5504 by mail, a mail renewal with Form DS-82, or Form DS-11 in person.
  3. Get the original or a certified copy of your name change document; a plain photocopy is not enough.
  4. Fill in the form for your route on a computer and print it. Sign Form DS-5504 or DS-82 before you mail it; sign Form DS-11 only in front of the acceptance agent.
  5. Get one passport photo that meets the official rules (see Passport photo requirements).
  6. For a mail route, put the form, your most recent passport, the name change document, the photo and any fee in one envelope and mail it, preferably with tracking. For Form DS-11, go to an acceptance facility in person.
  7. Watch your email for status updates and keep your mailing receipt.

  • Don't mail Form DS-11. You hand it in, unsigned, at an acceptance facility.

These are government fees. They are paid to the U.S. Department of State and, for Form DS-11 only, to the acceptance facility. They are not paid to Passmere.

By mail or at an acceptance facility, pay the application fee with a check or money order payable to "U.S. Department of State", with the applicant's name and date of birth in the memo. The acceptance fee is paid separately to the facility. Online renewals are paid by credit or debit card.13

How much does a name change cost?

On a recent passport, with Form DS-5504 and routine service, the government charges no fee1. Expedited service costs $604. After more time has passed, you pay the usual renewal or new passport fees shown above.

Can I send a photocopy of my marriage certificate?

No. Send the original or a certified copy. It comes back to you in a separate mailing.

My name is misspelled in my passport. What do I do?

That is a data error, not a name change. Mail Form DS-5504 with the passport, one photo and evidence of the error, as explained in "Fixing a data or printing error" above.

I use a different name but never had a court order. Can I update my passport?

There is a route for that: you apply in person with Forms DS-11 and DS-60 and bring public records that show you used the new name. If your case is unusual, check the official source before you apply.

Will Passmere ask for my Social Security number?

No. The official passport forms ask for your Social Security number if you have one; if you were never issued one, you enter zeros and attach the signed statement the form describes.18 Passmere never asks for it: you write it on the form yourself.
